The codes that apply to home care
These are the codes most often used for home health and personal care payroll in NCCI states.

Splitting clerical and outside sales payroll
Most states allow clerical payroll to be separated into 8810 and outside sales into 8742 — but only when the employee performs no duties in the higher-rated class and the payroll is separately recorded in your books. An office manager who occasionally fills a shift generally must be reported entirely at the caregiver rate.
California is different
California uses WCIRB classifications rather than NCCI. Home care payroll typically falls under 8827 (home health care and home infusion) with 8810 available for clerical. If you operate in California and your policy shows 8835, that is a sign the policy was written on out-of-state assumptions and should be reviewed.
How miscoding shows up
Watch for these red flags on your declarations page or audit:
- •All payroll in one code with no clerical split
- •A facility code (8829) used for an in-home-only agency
- •Experience modification applied to the wrong entity or FEIN
- •1099 caregivers excluded from payroll but uninsured — they get added at audit
- •Overtime reported at full pay rather than straight time where the state allows the exclusion
