Wording that causes rejections

A certificate gets bounced back for predictable reasons:

  • Certificate holder name or address does not match the contract exactly
  • Additional insured requested but no endorsement form number referenced
  • Waiver of subrogation missing on workers' compensation
  • Primary and non-contributory language required but not shown
  • Coverage listed that the policy does not actually include
  • Notice-of-cancellation days that exceed what the carrier will issue

What to send your broker

Attach the insurance requirements exhibit from the contract, the exact legal name and address of the certificate holder, the required endorsements, and the effective date. With those four items, KTL can normally issue a compliant certificate the same business day.

State home care licensing agencies often want a certificate naming the state agency as holder with proof of a surety bond and employee dishonesty coverage — not additional insured status.
